Partner In Care offers In-Home Support to maximize the independence and functional skills of the participant. Partner In Care assists the participant to work towards their goals.

In-Home support may include:

• Personal care support
• Assistance with mobility/transfers
• Assistance to attend appointments
• Assistance with eating and drinking
• Medication assistance
